Confidentiality, Mental Health Records & Release Policy
Imperial Foundation Community Development Center (IFCDC) protects all mental health, medical, and wellness-related records with the highest level of confidentiality and restricted access.
Executive Custodian
All protected health information is managed under the authority of Mr. Fahreal Allah – Owner & Executive Director. No records are accessed, shared, or released without his authorization, except where required by law.
Client Rights
- Your information is confidential.
- You may request a copy of your records.
- You may authorize IFCDC to share your information with another provider or agency.
- You may revoke that authorization in writing at any time.
How to Request or Release Records
To request records or authorize IFCDC to share information with another provider, please contact us using the secure channels below. A Release of Information (ROI) form is required and must be approved by the Executive Director before any records are shared.

Security & Access Controls
IFCDC uses role-based access controls, encrypted storage, and strict internal protocols to ensure that only authorized personnel can view limited information needed for their role. Staff cannot independently release or export records.
